This form is required to initiate a Return Merchandise Authorization (RMA) for an Optoma projector used in the Brunswick Spark system.
Before Submitting:
Please ensure that the issue has been diagnosed by Brunswick Technical Support and that you have obtained a valid Ticket Number. Submitting an RMA without prior support and diagnosis may result in delays.
Important:
- All fields below must be completed.
- If a field does not apply, please enter “N/A” to avoid delays in processing.
Once submitted, please allow up to four (4) business days to receive your RMA number and shipping instructions.
Shipping Note:
- The center is responsible for shipping the projector to Optoma.
- Optoma will cover the return shipping once the repair is complete.
