The information on this form is required to request an RMA repair for the Optoma projector used in the Brunswick Spark system.
Please make sure you have diagnosed the issue with Brunswick Support and obtain a Ticket Number for the issue. Submitting an RMA without initial diagnosis may result in delays in authorization.
Once the form is submitted it will take up to four (4) days to receive your RMA number and shipping information.
The center is responsible for the shipping fees to send the projector to Optoma for repair, Optoma will cover return shipping.
