Optoma Projector RMA Request

This form is required to initiate a Return Merchandise Authorization (RMA) for an Optoma projector used in the Brunswick Spark system.

Before Submitting:

Please ensure that the issue has been diagnosed by Brunswick Technical Support and that you have obtained a valid Ticket Number. Submitting an RMA without prior support and diagnosis may result in delays.

Important:

  • All fields below must be completed.
  • If a field does not apply, please enter “N/A” to avoid delays in processing.

Once submitted, please allow up to four (4) business days to receive your RMA number and shipping instructions.

Shipping Note:

  • The center is responsible for shipping the projector to Optoma.
  • Optoma will cover the return shipping once the repair is complete.
Please enter the valid serial number located on top of the Spark node or on the projector itself. Inaccurate or missing serial numbers may result in delays processing your RMA request.
